Yep, Hawaii is in a crisis and getting worse everyday. Yesterday we had 1068 new covid cases and 4 new deaths. The Gov still won't take action b/c he worries about the economy. However, people are cancelling their trips to Hawaii anyway. Visitors came here because of the low number of covid cases. Not any more!
Meanwhile at home, today's new cases are 43 adults and 20 children (YIKES!), and all are residents. From March 2020 to August 2021, we lost 2 residents to the virus. From August 1 to September 2, we lost 6 more. There are two new clusters, both religious gatherings, total of 4 churches that created clusters. The press release also said: "This holiday weekend, our choices and actions will determine how much suffering this island will have to endure in the month to come." I hope people take that to heart.
That was followed by another release. The county’s 2 free test sites are testing a total of over 700 people per day. That's good news. There are 7 other test sites and one that comes to your home. Most of them are free. Everyone is urged to be tested. Info was provided with addresses, phone numbers, days & times for tests, and whether they are walk-in or by appointment. One is not far from my home - a drive-in test site. I'm going tomorrow. (With Ted, of course.)
I think our mayor needs to impose rules - like insisting on proof of vaccination before entering a restaurant or bar and limiting group capacity - especially in beach parks. Technically, he can't because the governor overrules the mayors. However, the Oahu mayor said screw that. He ordered that customers at restaurants, gyms, movie theaters, museums, and other businesses will need to show proof of vaccination or a negative COVID-19 test within the last 48-hours to enter the businesses beginning 9/13. Employees who are not vaccinated will be required to test weekly.
He's also threatening mandatory vaccination if Oahu covid numbers don't drop. (I don't think he can enforce that, but I give him a anyway.)
The Lt. Gov. tried to get the governor to order a shutdown for the Labor Day weekend to give the hospitals a break, but the gov refused. The only thing that will be done is Oahu & state police will spend the long weekend breaking up any large gatherings.
You can't wait until life isn't hard anymore before you decide to be happy.

The rise in people using ivermectin, an anti-parasitic drug usually reserved for deworming horses or livestock, as a treatment or preventative for Covid-19 has emergency rooms “so backed up that gunshot victims were having hard times getting” access to health facilities, an emergency room doctor in Oklahoma said.
This week, Dr. Jason McElyea told KFOR the overdoses are causing backlogs in rural hospitals, leaving both beds and ambulance services scarce.
“The ERs are so backed up that gunshot victims were having hard times getting to facilities where they can get definitive care and be treated,” McElyea said.
“All of their ambulances are stuck at the hospital waiting for a bed to open so they can take the patient in and they don’t have any, that’s it,” said McElyea. “If there’s no ambulance to take the call, there’s no ambulance to come to the call.”

I'm sure ERs must be using some type of triage, but, really, maybe it's time to triage non-vaxxed and ivermetin patients to the end of the line.

Maybe tomorrow I'll find some time to research how this horrific outbreak has affected our tourism dollars. We're also fighting red tide and green algae so it may be hard to separate the two. We have some French neighbors who haven't been able to come down for over a year. Hospitals have been rationing care for weeks. They've done so by postponing non-emergency surgeries.
Some hospitals may soon be triaging based on predicted survival. Because very few vaxed are dying they're probably less likely to need vents. They will probably be given care ahead of non-vaxed because they are less likely to need ICU beds.
Those needing ambulances to get to the ER are, in a way, triaged already. Those patients are already severely ill and can only be treated by EMTs until a bed opens. Those patients wait in the ambulance rather than getting timely care in the ER. This ties up EMTs and ambulances and sicker patients aren't getting even EMT care in a timely manner.
Hawaii is running out of supplemental oxygen. If hospitals have to ration oxygen, they are likely to give it to patients most likely to survive. One reason? Patients on vents and ECMO are using up most of the oxygen. Those on vents for covid typically have a survival rate of 70-80%.* ECMO is a hail Mary with covid; survival rate is approx 45%. ECMO requires that 100% of a patient's blood volume be oxygenated because their lungs can't absorb oxygen anymore.
Patients needing nasal cannulas to increase their oxygenation aren't requiring nearly as much oxygen as sicker patients.
But hospitals can't and won't take patients off vents or ECMO. Hospitals will have to limit the number of ICUs available to new admissions.
But there's another layer of triage. A large majority of ECMO patients have to be treated at major medical centers. Those centers are full.
Triage for critically ill covid patients now happens based on the number of ICU beds and ICU staff. It's easier to find beds for those who don't need ICUs. I'm assuming the vaxed are less likely to need ICU beds.
And, hospitals can create more regular hospital beds for covid patients by setting up mobile hospital tents. ICU beds can't be in mobile hospital tents. Most nurses and doctors aren't trained to provide ICU care.
Triage is already happening.
*In 2020, the survival rate for vented patients was 20-30%. Treatment for covid patients on ventilators has improved.

To update on Hawaii, oxygen arrived in Hawaii today, with more coming on Wednesday.
Our governor finally did ... something. He changed the way violators are fined. It had been a misdemeanor, so the Courts were clogged and public defenders were exhausted. Now it's a fine - up to $250 for individuals for violating limits for gatherings and up to $500 for businesses and event coordinators. My island and Oahu chose the highest fines.
So who's going to write up the fines? Not enough police for that, so county park rangers and liqueur commission investigators are also participating.
I guess we'll know how that worked out on Tuesday and how many new cases we have later in the week.

I suspect the local reporting got what the doctor was saying wrong. Watching the snippets and reading the article, I'm guessing that he was talking about the generic dangers of ivermectin at one point, and at another point was talking about how people sick with covid were tying up the hospitals, perhaps linking reliance on ivermectin with refusal of vaccines and how that refusal was causing the overload of patients.
