This is why every lift that takes place on my employer's property requires a certified rigging plan. (The crane in this instance didn't have extra ballast behind the cab. Also I question the adequacy of the outriggers)

Even we office drones who will never enter a production area are required to be certified and tested annually on knowledge of basic industrial safety procedures, including the need to file a rigging plan, the need to validate that all rigging equipment is inspected before a lift and the need to ensure that the rigging plan ensures adequate personnel to cordon off the lift path to make sure someone doesn't wander underneath a load.

They don't tell us enough to fill out the rigging plan form or to actually run a crane, but they do want us to be able to spot when someone is not following procedures.

Just looking at the river bank, I would say that is some seriously soft and possible wet soil. I have little idea of how these things work but unless the footprint was made larger, the front outriggers could earlier dig into soft soil.

There's a large furniture store a few miles from me. About 5 years ago I was driving home and drove by it and noticed a truck with its front end up in the air. It was installing new air conditioning units on the roof and obviously hadn't figured out the shift in the center of gravity. The truck's crane had landed on the building's facade and damaged it, but I really wonder about the heavy air conditioning unit and the spot on the roof that it landed on. :shock:

Little oopsies when air conditioning units slam into the roof can cause big oopsies later. Check out the story of the Sampoong Department Store collapse in Seoul in 1995, where over 500 people died and almost 1000 were injured. Cracked out the side frame fittings by the look of the pics, major piece of the structure, and certainly would cause center wing box damage. Being a 400 mod, yeah, write off and collect the insurance money...

An Amtrak train bound for Iowa derailed near Mendon, Missouri with injuries reportedly after hitting a dump truck on the tracks. Lots of coverage in the usual places.

Dump truck on the tracks. Three dead, including truck driver. It was the Southwest Chief.


Edited to add: Apparently, from at least one witness on the train, the truck was trying to race the train at the crossing.

At least 12 people have died and 251 were injured in a chlorine gas leak from a storage tank at Jordan's Aqaba port, as authorities called on residents to shut windows and stay indoors.

Key points:
A tank filled with 25 tonnes of chlorine gas fell while being transported
The port's grain silos halted work and ships were asked to avoid the area
Aqaba port is a major transit route for Iraqi imports and exports
